Iranian President Masoud Pezeshkian reportedly suffered minor leg injuries during an Israeli airstrike last month targeting a secret underground facility in Tehran.
The attack took place on June 16 during a high-level emergency meeting of the Supreme National Security Council, according to Fars news agency, which is linked to the Revolutionary Guard.
The report claims that six bombs were dropped, damaging all known access and exit points of the facility and disrupting its power and ventilation systems.
The president and several officials are said to have escaped through an emergency shaft, with Pezeshkian sustaining slight injuries to his leg during the escape. He later reached a secure location.
Videos from social media at the time showed multiple strikes in north-western Tehran during the brief war between Iran and Israel.
It is now believed that one of these attacks was aimed at the underground site hosting Iran’s top security officials.
Israel has not confirmed or denied the report, and it remains unverified by independent sources.
However, Iranian authorities have since begun investigating possible internal leaks or the presence of Israeli agents inside the country.
The strikes reportedly created panic among Iranian leadership. Israel had earlier targeted and eliminated several senior figures within Iran’s Islamic Revolutionary Guard Corps and military at the start of the conflict.
Iranian officials acknowledged that they were caught off guard and faced delays in decision-making within the first day of the attacks.
While Israel stated that Ayatollah Ali Khamenei was also a target, his whereabouts became unknown after he was relocated to a secure location disconnected from communications.
Earlier, on June 13, Israel launched a wide-ranging assault on Iranian nuclear and military sites, claiming it was to stop nuclear weapon development.
Iran responded with its own aerial attacks but continues to insist that its nuclear program is for peaceful energy use.
Later, on June 22, the United States followed with its own coordinated air and missile strikes against three Iranian nuclear facilities.
US officials offered varying views on the damage, but President Donald Trump claimed the facilities were completely destroyed.
